Riassunto

Gli standard di qualità dei servizi per tossicodipendenti

 

Guus van der Upwich di Brema , è un rappresentante del gruppo di lavoro "Standard di qualità e controllo della qualità" di ERIT Europa, che si occupa di studiare e definire quali devono essere gli standard in grado di rendere il prodotto del lavoro, delle organizzazione che si occupa dei problemi di abuso e dipendenza, qualitativamente elevato.

Secondo Upwich il processo di controllo di qualità si articola lungo la valutazione dei diversi aspetti che caratterizzano le fasi del lavoro di un servizio che opera nel campo della tossicodipendenza.

Sono identificabili alcune categorie che devono soddisfare particolari criteri per avere un lavoro qualitativamente elevato, inoltre tali criteri devono essere confrontati con uno standard di misura definito a priori. La qualità deve essere costantemente monitorata. I tre criteri fondamentali definiti da ERIT sono competenza, qualificazione ed esperienza.

Upwich afferma che per procedere al Change Management (cambio del modello del processo) è essenziale evidenziare i punti deboli e i punti forti di una organizzazione. Durante questo processo l'azienda deve affrontare la gestione dell'instabilità, facendo affidamento sugli operatori che possiedono il pensiero trasversale. Il change management si realizza destabilizzando pezzo per pezzo il sistema, ponendo a priori delle regole precise, e solo nel caso in cui l'organizzazione sia in "buona salute".

Guus van der Upwich de Brema est un représentant du group "Quality Standards and Quality Control Sistem" de ERIT Europe. Ce group s'occupe d'étudier et définir les standards qui peuvent rendre le produit du travail des organisations qui s'occupe des problèmes des toxicomanes vraiment de qualité.

Upwich dit que le processus pour la vérification de la qualitè prend sa forme en utilisant l'évaluation des plusieurs aspects qui décrivent les phases du travail des services pour les toxicomanes.

On peut identifier chaque catégorie, qui doit satisfaire des critères particuliers pour avoir enfin la garantie de qualité. Ces critères doivent être comparés avec un standard de misuration qui est établi au début. La qualité doit être tout le temps vérifiée.

Trois critères ont été choisis par ERIT comme les plus importants: compétence, qualification et expérience.

Upwich parle aussi de la procédure de "Change Management" (changement de modèle de processus). D'abord, pour faire ce changement, il faut souligner les points forts et faibles de l'entreprise et en plus l'organisation doit gérer l'instabilité s'en remettant aux opérateurs qui possèdent la pensée transversale.

On peut réaliser le "change management" en destabilisant le système, seulement si on a choisi au début des règles exactes et seulement si l'organisation est actuellement "en bonne santé".

 

Presentazione della giornata di lavoro.

Il Dipartimento Dipendenze Patologiche dell'Azienda USL di Reggio Emilia ha organizzato una serie di incontri aventi come tema "Gli Standard di Qualità dei servizi per tossicodipendenze".

Nell'ambito di questo percorso si è svolta la giornata di lavoro del 14 maggio 1999, che ha visto la presenza del dr. Guss van der Upwich di Brema, rappresentante del gruppo di lavoro dal tema "Standard di Qualità e Controllo della Qualità" della federazione ERIT Europa, progetto speciale che gode del supporto della Unione Europea.

Il gruppo di ERIT Europa da tempo si occupa di studiare e definire quali devono essere gli standard caratterizzanti un'organizzazione che si occupa dei problemi di abuso e di dipendenza in modo qualitativamente elevato. Il processo di controllo di qualità invece, si articola lungo la valutazione dei molteplici e differenti aspetti del lavoro svolto dai servizi che operano nel campo della tossicodipendenza. ERIT Europa offre così la possibilità a tutti gli operatori che si occupano dalla tossicodipendenza, di inserirsi in questo percorso di lavoro europeo.

Guus van der Upwich si occupa inoltre di consulenze rispetto la qualità e il "Change Management" per le organizzazioni industriali, per le organizzazioni sanitarie, e per tutte le aziende definite "No Profit" in Germania, che desiderino riflettere sui possibili principi di cambiamento del modello del processo.

Il lavoro sulla qualità è partito nell'Azienda USL di Reggio Emilia qualche anno fa, costituendo inizialmente dei "Gruppi di miglioramento", per procedere poi con "Circoli di qualità", "L'accreditamento sperimentale", e infine ultimamente la "Certificazione di qualità" partita formalmente all'inizio dell'anno 1999 e che prevede una durata di 16-18 mesi.

Da qui l'idea di organizzare un incontro volto ad avvicinare la realtà di Reggio Emilia, e di altre zone d'Italia, all'esperienza del gruppo che, a livello europeo, si occupa di qualità.

 

Il lavoro del gruppo di ERIT Europa

 

La prima distinzione che occorre fare se si parla di "Percorsi di qualità " è definire la differenza che esiste tra criterio (o indicatore) e standard. Il criterio (o indicatore) è una caratteristica desiderata, e che deve quindi essere soddisfatto per avere la qualità, mentre lo standard é il metodo che si usa per misurare il criterio. Le prime tre tabelle seguenti mostrano le varie categorie che devono soddisfare diversi criteri, per rendere qualitativo il lavoro di un' organizzazione.

Dei criteri costituenti ogni categoria, alcuni sono indispensabili, sono il "cuore" dello standard e sono state definite come vincolanti (n° of core) mentre gli altri sono i criteri correnti, usati da diversi Paesi per definire una categoria (n° of common).

Le altre tabelle analizzano più in specifico le 12 categorie e i loro vari sotto punti, indicando i vari criteri con alcuni esempi specifici.

 

ANALISI DI ALCUNE CATEGORIE RIPORTATE NELLE PRIME TRE TABELLE:

Le risorse umane (2) : questo ambito si divide in sotto categorie tra cui le risorse umane e il management (2.3) con cui si sottolinea che sono necessari tutti i cambiamenti atti ad agevolare le persone che operano all'interno del servizio, e che è necessario il training plan (2.4) cioè la formazione programmata che deve essere fornita al personale.

L'accesso o accessibilità (3) serve per procedere ad un cambiamento di modello che fornisca al cliente un servizio che sia per lui soddisfacente, è quindi necessario che non sia il cliente a doversi adattare al servizio.

L'ammissione o ammissibilità (4) riguarda il come le persone vengono accolte, cioè l'accoglienza. L'assessment (4.1) è una procedura che è utilizzata per operare una selezione tra le diverse opportunità di cura disponibili. Nel momento in cui i clienti sono accettati si valuta se i motivi per cui sono stati accolti sono gli stessi per cui si sono rivolti al servizio.

Il care policy and approach (5): questa categoria si articola in il planned care (5.1) cioè la cura prevista così come è stata pianificata. In base al planned care ci si domanda se la cura è stata pianificata in base a degli standard, se è flessibile e se si adatta all'utente. Si sottolinea anche che l'utente sia coinvolto durante la pianificazione della cura. L'organizzazione che possiede dei buoni standard e dei buoni criteri adatta sempre la terapia al cliente ed è questa la migliore diagnosi che il servizio può fornire all'utente. La terapia non è sempre un processo in salita, al suo interno ci sono anche ricadute, perché bisogna accettare il concetto per cui la crisi (la ricaduta) è un nuovo cambiamento, un possibile miglioramento. Il care review (5.3) è una retrospettiva di analisi effettuata in diversi intervalli di tempo, per verificare se ciò che era stato pianificato procede come dovrebbe da programma.

Gli interventi (6) servono per valutare come viene portato avanti il lavoro, così da poter trasformare ciò che non funziona come dovrebbe. La consulenza e il trattamento psicoterapeutico (6.1) devono basarsi su procedure definite per iscritto e verificabili dall'esterno. Ci si deve chiedere quale sia l'agire psicoterapeutico qualificato, in quanto esistono vari spettri sulla definizione di setting psicoterapeutico. In Germania ad esempio questo dibattito è molto acceso ed è stata messa a punto la legge sulla psicoterapia, che riconosce solo ad alcune scuole la possibilità di fare psicoterapia (es. la psicoanalitica e la comportamentale). Mentre per quanto riguarda chi si occupa di disintossicazione c'è nella sanità tedesca un temporaneo congelamento, si sta pensando di coinvolgere solo l'apparato medico in questo compito, e quindi gli operatori sociali non possono portare avanti il loro lavoro. E' necessario che ci sia una regola per quanto concerne la prescrizione del metadone, e ci devono essere regole scritte anche per quanto riguarda la disintossicazione.

La case closure (7) è la conclusione del ciclo della terapia in cui si fa il punto di ciò che è stato fatto e se è necessario si passa ad trattamento supplementare. L'importante è stabilire i criteri per un passaggio indolore a questo trattamento alternativo.

La documentazione (12) è la descrizione esatta di ciò che si sta facendo, viene qui descritto il ruolo di ogni professionista in un determinato passaggio del processo, di cui lui diviene il diretto responsabile.

E' necessario specificare che in ogni ambito definito nelle tabelle ci sono tante categorie, alcune delle quali sono state appena definite, che riguardano gli standard ai quali sarebbe necessario attenersi, e inoltre ci sono i criteri sui quali basarsi.

 

E' molto difficile giungere ad una definizione così chiara e schematica di indicatori della qualità, molto spesso infatti nelle organizzazioni invece di definire gli standard qualitativi si va ad osservare, e quindi a costruire una lista di criteri poco importanti, ne sono un esempio la registrazione del n° di volte che è vista una persona, tralasciando magari di definire cosa è stato fatto durante l'incontro.

La qualità deve essere costantemente controllata e monitorata, per poterla migliorare sempre di più.

Per ottenere una migliore qualità è anche necessario incoraggiare i dipendenti dell'azienda, e stabilire una salda cooperazione con altre associazioni esterne che si occupano delle stesse problematiche.

E' importante qui sottolineare che ogni Paese europeo deve definire quali criteri saranno comuni e vincolanti per tutti, lo standard sarà poi definito individualmente da ogni Paese, in base alla sua particolare esperienza. Ad esempio se viene definito come criterio fondamentale per tutti i Paesi "la pulizia delle strutture", questa deve essere fatta per fornire un servizio di qualità, ma ogni Paese può decidere il suo standard, cioè ad esempio la frequenza con cui si chiamano gli addetti alle pulizie.

Se si fosse prescritto uno standard uguale per tutti in Europa, non si sarebbe mai raggiunto un accordo. Infatti il gruppo di ERIT Europa ha posto i tre criteri fondamentali di COMPETENZA, QUALIFICAZIONE ed ESPERIENZA, senza però voler entrare in merito alle diverse legislazioni nazionali. Lo standard che ogni nazione ha dipende dalle proprie leggi, ma è necessario che l'azienda si sforzi di portarlo al meglio che si desidera, in base ai criteri descritti precedentemente, auspicando che alla fine ci si possa trovare con il medesimo standard in Europa.

Inoltre una necessità vincolante di ogni azienda, è possedere una strategia di budget ben delineata.

 

Change management o cambio del modello del processo

 

Vengono presentati tramite una simulazione multimediale la qualità degli operatori e la qualità dei processi svolti. Questa simulazione al computer mostra come con le interviste si riesca a delineare, tramite la visione soggettiva di ogni operatore, un quadro chiaro rispetto ciò che ognuno percepisce sull'azienda.

Per procedere al cambiamento di modello è essenziale evidenziare i punti deboli e i punti forti, eliminando poi il più possibile i problemi.

Un punto che dovrà affrontare l'azienda lungo questo percorso è la gestione dell'instabilità, su cui è necessario intervenire attraverso una valida motivazione: Per gestire la instabilità è necessario fare affidamento sugli operatori che posseggono il cosiddetto pensiero trasversale, e che difficilmente sono accettati positivamente, in quanto sono considerati dei disturbatori dei "bastian contrari", ma possono anche essere visti contemporaneamente come operatori all'avanguardia. Sarebbe quindi produttivo per le grandi organizzazioni aprire le porte a questi operatori.

La maggior parte delle organizzazioni è orientata alla stabilità, infatti esistono diversi miti intorno ad essa, ma che crollano nel confronto con l'instabilità. Il primo mito è quello creativo (non esiste ordine senza ordinatori) ciò non è vero nella realtà, infatti ne è un esempio la natura che crea nuovi ordini senza che però esista un ordinatore. Il secondo mito è quello evolutivo (il cambiamento è un processo continuativo) in natura però va a salti. Il terzo è il mito della causalità per cui ogni cambiamento ha la sua causa, mentre ciò non è vero nell'instabilità, infatti in questo caso non esiste una sola causa ad un processo ma molteplici. Infine nella stabilità esiste il mito dell'equivalenza per cui l'effetto é proporzionato alla forza della causa. Questo mito collude con la situazione reale di instabilità, si pensi ad esempio alla teoria del caos, della farfalla che sbatte le ali a Tokio e provoca la pioggia in Italia.

L'instabilità non diventa caos se ci si è prevenuti stabilendo inizialmente delle regole, un inquadramento. Nel sistema si può pensare al cambiamento nel momento in cui si è inquadrata la situazione, e si sono poste delle regole.

E' possibile sfruttare le dinamiche del sistema per vedere se si è raggiunto il livello di cambiamento a cui si voleva arrivare, e questo è un controllo che differisce dal normale controllo dall'alto al basso. La modificazione del sistema procede allora con un movimento ondulatorio e non con una crescita costante in linea retta. Un limite degli uomini è che si accorgono del cambiamento solo quando è già avvenuto e non mentre sta avvenendo.

I processi di cambiamento partono quando dall'alto danno la possibilità di farlo, o partono da coloro che danno il ritmo. E' importante quindi scoprire chi, dove e come in un'organizzazione dà il ritmo; ad esempio in ospedale spesso non è il primario che dà il ritmo, ma è la capo infermiera. Bisogna quindi individuare a chi rivolgersi per cambiare il ritmo, l'importante era avere stabilito le regole a priori, dopo di che ogni cambiamento del ritmo che insorge crea nuovo ordine e non caos.

L'instabilità permanente distrugge l'organizzazione, non bisogna infatti portare il caos in tutti i casi , ma lo si dovrebbe avere solo durante il processo di cambiamento.

Le organizzazioni stabili uccidono la creatività e difficilmente arrivano a comprendere i cambiamenti dei processi.

Sarebbe opportuno avere un'azione decentralizzata, lasciando più indipendenza al sistema, con la definizione delle regole chiare che accompagnino in ogni momento l'effetto di cambiamento sul sistema.

Anche la crisi è uno spunto per il cambiamento, l'instabilità critica non è altro che una opportunità di trasformazione ; è necessario tuttavia in questo caso, non creare instabilità nella intera organizzazione, pena il suicidio della stessa, ma bisogna piuttosto procedere per cambiamenti piccoli di solo alcune parti del sistema. L'ideale sarebbe attuare il change management quando l'organizzazione "sta bene", ed è in grado di sopportare le trasformazioni.

L'errore che non va a scapito dell'utente deve essere accettato e tollerato. Importante è però agire considerando l'oggi, senza troppo rivolgersi agli errori o ai traguardi del passato, questo non farebbe altro che aumentare la vulnerabilità dell'azienda.

I sistemi divengono intelligenti quando permettono ad ognuno dei suoi operatori di avvalersi dell'intelligenza altrui, che può essere "sfruttata" in un sistema di rete. I sistemi che al contrario non sono aperti, e non permettono la circolazione delle informazioni, rimangono statici ed è difficile operare in loro il cambiamento. Fare circolare le informazioni che si hanno a disposizione rende intelligente l'altro, perché quest'ultimo può usarle trasformandole, ed apportando così attraverso loro una parte della sua specifica creatività all'intero sistema.

L'organizzazione deve essere disposta ad accettare il cambiamento continuo delle persone al suo interno, cercando di utilizzare al meglio le loro caratteristiche specifiche. La combinazione che si è rivelata la più sensata del personale all'interno di un sistema, è quella che mette insieme le persone più stabili e quelle più creative.

La concorrenza interna all'organizzazione andrebbe fatta solo nel caso in cui serva per il processo di change management, in caso contrario si rischierebbe la distruzione dell'organizzazione stessa ; nelle aziende in cui gli operatori sono in concorrenza tra loro il cambiamento attuabile è molto piccolo.

Per capire il sistema è necessario osservarlo da una prospettiva esterna, che è la più utile per fornirne una visione d'insieme.

In conclusione si può riassumere che per ottenere un cambiamento del modello del processo di un'organizzazione, bisogna cominciare destabilizzando pezzo per pezzo il sistema, ponendo a priori delle regole precise, avendo fiducia nel fatto che il sistema possa diventare creativo sfruttando le potenzialità delle persone che vi operano. Bisogna ancorare, cioè dare un chiaro messaggio agli operatori con il pensiero trasversale, dove si mette in luce la loro importanza nell'organizzazione. Per scoprire chi batte il ritmo nelle organizzazioni bisogna disturbare il sistema, le gerarchie. Tutto il processo di cambiamento può venire solo dall'interno del sistema, dall'esterno può venire solo un supporto a tale cambiamento, uno spunto perché il sistema impari a utilizzare il potenziale creativo che ha al suo interno. Importante è ricordare di non creare instabilità nell'azienda in crisi, ma solo quando esse è in buona salute.
